Nonce reuse

Some encryption schemes are secure only if a particular value is never used twice with the same key, and reusing it can be catastrophic rather than merely weakening.

It is not a gradual degradation. Depending on the scheme it can expose the plaintext or the key outright. Because it is a subtle implementation requirement rather than a visible configuration setting, it is a classic example of cryptography failing at the edges rather than at the algorithm.
