Reporting culture
Punishing mistakes buys silence, and silence is what turns a contained incident into one discovered three months later.
The person who clicked is the only one who knows, and whether they say so within five minutes or never is determined entirely by what they expect to happen to them. Every organisation says it wants early reporting; rather fewer have made it safe.
